Do you center the cupola, or put it towards one end? How far from the end? URLs showing examples appreciated!

They are not usually incorporated in looks! They are a design parameter of the building!

thanks, I assumed that. However, all the sites checked (about 100), only address ascetics with an occasional reference, without data, to ventilation. So I am stuck, for now.
